Your service area is already on file. This is how you measure against it.

A CCBHC application describes the boundaries of the geographic area the clinic serves, and that description has to align with a map submitted as a required attachment. The map is a deliverable of the application. Measuring against it is not.

That gap is the whole of our work.

What the application commits you to

A named location and a described boundary

FY2026 applications must include the name and address of the main clinic location and a description of the boundaries of the geographic area the CCBHC serves, aligned with a required map attachment. Once filed, that boundary is the standard your access and outreach are judged against.

The described area
Counties, ZIP codes, or a catchment boundary, written into the application and mapped.
The certification criteria
Availability and Accessibility of Services covers timely and meaningful access, outreach and engagement, and acceptance of all patients regardless of ability to pay or place of residence. Geographic reach is a certification matter, not a preference.
The renewal problem
The map is written once, for the application, and then never measured against. At renewal the outreach-and-engagement criterion has no evidence behind it.
Timing

A new cohort is about to be named

SM-26-014 carries $94 million with 94 expected awards at up to $1 million each. Applications closed 17 August 2026. Every clinic in that cohort has a service-area map on file and an outreach-and-engagement criterion to satisfy, starting now.

If you applied, the audit turns the boundary you described into a measured baseline inside the first quarter of the award. If you are certifying under a state pathway instead, the criterion is the same and so is the work.

Measurement fit

Access is an engagement metric. Community education is not.

Most CCBHC outreach is engagement work, which is the track our measurement was built for. Where a clinic also runs community prevention or anti-stigma programming, that piece is measured on the second track. We agree which applies per campaign before anything is produced.

Engagement track

Care navigation, crisis line promotion, treatment access, and school or justice referral pathways. Measured in qualified referrals, enrollment conversion, and time to first tracked response, reported against the described service area.

Prevention and awareness track

Community education, stigma reduction, and naloxone or resource distribution. Measured in priority segment reach, message recall against a baseline, resource uptake, and partner activation.

Both tracks report coverage against the boundary in your application, which is the number certification actually asks about.

What you buy

The system, by module

Four modules that run in sequence. Buy the first and decide from there.

Module 01 — Reach Audit
A map of your service area against where eligible people actually live, a baseline cost per qualified referral, and a message risk register. Three weeks. From $8,500.
Module 02 — Message System
Audience-specific message architecture, safe-messaging review, and a production kit you keep. $22,000 to $34,000.
Module 03 — Local Deployment
Localized assets and partner activation across the service area, including Remote Studio production. $16,000 to $28,000 per jurisdiction per year.
Module 04 — Measurement and Optimization
Live dashboard, a monthly working session where budget moves toward what is producing referrals, quarterly narrative written in grant language, and an annual Outreach Evidence File. $1,500 to $2,800 per month.

Bundled first year: $58,000 to $86,000 for Modules 01 through 04. Smaller awards start at $24,000 to $32,000 using an existing regional campaign core. Media spend is separate and paid directly to platforms.

Behavioral health outreach against a bounded population

Our published cases are VA behavioral health engagements. The population differs from yours; the structure does not. A defined service area, a named eligible population, clinical review before release, and an outcome verified by someone other than us.

Clinical safety

No PHI, and nothing released without review

No protected health information
We do not receive, store, or process PHI. Measurement runs on source-tagged intake counts and tracked response data, not patient records.
Documented review
Every asset passes a documented safe-messaging review before release. We publish the pass rate as a reported metric.
Named limits
Where a campaign addresses a clinical question we have not worked in, we say so and recommend someone who has.
